ABSTRACT

Objective To investigate the clinical effect of microsurgical repair after ring avulsion injuries. Methods From March, 2009 to December, 2014, 6 cases (6 fingers) of ring avulsion injury were repaired. There were 4 males and 2 females with an age range of 18-30 (mean, 22) years, which were 4 cases of ring finger and 2 cas-es of middle finger. The plane of injury was metacarpophalangeal band. Fracture and dislocation was reduced and fixed if damage fingers following with fracture and dislocation of interphalangeae.The damage ligament and volar plate and extensor tendon was repaired. The digital arteries and veins were repaired, and the digital nerves were sutured. System rehabilitation exercise carried out postoperatively. Results All fingers survived, and were followed-up for 8 to 30 months. The appearance of the fingers was excellent. The avarege range of motion of the proximal interpha-langeal was 80°(ranged from 70°to 100°).The distal interphalangeal was 50°(ranged from 40°to 60°).The sensory re-covery of finger pulp ranged from S3to S3+.The two-point discrimination was 5 mm to 12 mm. According to the Eval-uation Criteria for Finger Replantation and Reconstruction Issued by Chinese Hand Surgery of Chinese Mdical Associ-ation, 4 fingers had excellent while 2 had fairly good function. Conclusion Microsurgical repair of ring avulsion injury can achieve satisfactory clinical results by rehabilitation exercises postoperatively.

ABSTRACT

Objective To investigate the method and result of repairing multi-fingers soft tissue defects using the dorsal metacarpal flaps with cutaneous branches as pedicle.Methods From February,2010 to January,2013,9 patients with multi-fingers tissue defects were treated with the 2nd,3rd,4th dorsal metacarpal flaps with cutaneous branches as pedicles.The area of flaps ranged from 1.2 cm × 2.5 cm to 2.5 cm × 5.0 cm.The donor sites were sutured with full thick skin graft.Results All flaps survived.After a followed-up of 8 months to 24 months(average 12 months),the texture and shape of the flaps were good and non-bloated.The flap sensibility as sessment were S3-S3+.The two-point discrimination testing were 10 to 13 mm (average 11.6 mm).The TAM score of range of motion was 60% to 75% of the healthy side.The skin graft of donor site were soft.Conclusion Procedure of dorsal metacarpal flaps with cutaneous branches as pedicles easy is a good method to repaire the soft tissue defects of muhi-fingers.

ABSTRACT

This study was aimed to discover the knowledge of urination formula-syndrome in theTreatise on Exogenous Febrile Diseasebased on the Sunshine diagram of multi-layer complex concept network express. A total of 39 items about urination formula-syndrome in theTreatise on Exogenous Febrile Diseasewere collected, and then regulated into standard expression. The database was established and the multi-layer complex concept network express was constructed. The Sunshine diagram was drawn and the connotation rules on urination formula-syndrome in theTreatise on Exogenous Febrile Diseasewere summarized through mode development of the diagram. The results showed that the Sunshine diagram collected 44 objects (i.e., formulas) and 191 properties (i.e. syndromes), which expressed the urination formula-syndrome visually. It was concluded that the application of Sunshine diagram in the formula-syndrome knowledge based on multi-layer complex concept network express provided certain references on the inheritance and development of classics in traditional Chinese medicine (TCM).

ABSTRACT

Aim: Our case report aims to inform practicing clinicians of an unusual presentation of vitamin C deficiency in the setting of a developed nation where this illness is rare and underappreciated. Case Presentation: We present the case of a noncompliant 16-year-old African American female with vertically transmitted human immunodeficiency virus/acquired immune deficiency syndrome who presented to the emergency department (ED) with a CD4 count of 4 and a hemoglobin level of 5.7 g/dL. In the ED, she was found to have persistent low-grade bleeding initially believed to be of an upper gastrointestinal origin, but which was later found come from the oral mucosa. Her stools were dark in color and guaiac positive. She was hemodynamically unstable, for which she was transfused with packed red blood cells and briefly treated with continuous norepinephrine infusion. Her initial coagulation studies were noncontributory with an international normalized ratio of 1.1, a prothrombin time of 35, and a platelet count that was also within normal limits. Esophagogastroduodenoscopy and a colonoscopy were both unremarkable. Bone marrow biopsy showed normocellular marrow with 80% cellularity and trilinear hematopoiesis. Her vitamin C level was zero. She was diagnosed with scurvy and treated with vitamin C supplementation. Discussion and Conclusion: Vitamin C deficiency can lead to an often-forgotten medical condition called scurvy. It can cause defective collagen synthesis leading to fragile capillaries, gingival bleeding, and cutaneous changes. Unrecognized, this condition can lead to significant bleeding and can be lethal in select patient populations. Our case is unique in that it shows that vitamin C deficiency can masquerade as upper gastrointestinal bleeding and may present with significant hemodynamic instability requiring blood transfusions and vasopressor support. It is therefore imperative to keep in mind the diagnosis of scurvy as a potential cause of hemodynamic instability even in an industrialized nation such as the United States. Vitamin C deficiency is a rare and underdiagnosed medical entity in the hospital setting that can lead to hemodynamic instability. Scurvy patients can present with melena and oral bleeding, mimicking hematemesis.

ABSTRACT

This study was aimed to analyze the regulation of syndrome-treatment pattern of classical Chinese medici-nal formulae for emotional diseases based on formal concept analysis. First, we dealt with the decision formal context of 51 prescriptions about emotional symptom in the Treatise on Febrile and Miscellaneous Diseases and the Es-sentials from the Golden Cabinet based on the principle of optimization. Then, we generated a new partial-order at-tribute diagram in order to present the specific character. Finally, we explained properties of partial-order structure graph from traditional Chinese medicine (TCM) experts' point of view based on knowledge discovery. The results indi-cated the relationship between prescription and syndrome of emotional diseases. It was concluded that method pro-posed in this paper worked well in treatment of description of syndrome differentiation and discovery of new knowl-edge from the known data in the clinical diagnosis.

ABSTRACT

Objective To investigate the methods and results of reverse island flap of the adjacent digit pedicled with the Y-V vascular of digital artery by anastomosis of superficial veins for repairing soft tissue defects of the fingers.Methods From March 2009 to June 2011,twenty cases with soft tissue defect distal to the proximal interphalangeal join of fingers were treated by reverse island flap of the adjacent digit pedicled with the Y-V vascular of digital artery by anastomosis of superficial veins.There were 12 cases of the index finger,eight of middle finger,the largest area of the flaps was 4.5 cm × 3.5 cm,and the smallest area was 3.5 cm × 2.5 cm,an average of the pedical length was 4.0 cm.All cases anastomosis one superficial vein,fourteen cases suture dorsal digital nerve,and the donor area covered with full-thickness skin graft.Results All flaps survived.Postoperative follow-up time ranged from 8 to 16 months,the appearance and texture of the flaps were excellent,the flaps with suture nerves,the two-point discrimination was 7 mm to 9 mm,the other flaps that the nerves were disconnected.The sensation of the flaps recovered to S2-S3,no morbidity of the donor fingers occurred.Conclusion Reverse island flap of the adjacent digit pedicled with the Y-V vascular of digital artery by anastomosis of superficial veins can form a longer vascular pedicle,to repair the soft tissue defect distal to the proximal interphalangeal joint,through anastomoses superficial venous can reduce the flap venous pressure obviously,improve the survival quality of the flap,the effect is satisfacted.

ABSTRACT

Objective To explore the clinical outcome of using modified great toe wrap-around flap to reconstruct degloved thumb and fingers.Methods Eighteen patients were involved.Based on different types of injury,four procedures were carried on for reconstructing degloved thumb and fingers:①Unilateral modified great toe wrap-around flap to reconstruct 9 degloved thumbs of distal proximal level and 3 degloved fingers of proximal interphalangeal joint level.②Unilateral modified great toe wrap-around flap with second toe medial flap to reconstruct 2 total degloved fingers.③Bilateral modified great toe wrap-around flap to reconstruct 2 thumbs.④Bilateral modified great toe wrap-around flap and second toe medial flap with neurolized super thin anterolateral thigh flap to reconstruct 12 degloved fingers.This wrap-around flap carried with entire nail.A triangle flap was reserved at medial plantar of great toe.Results All free flaps were survived in one stage.Fifteen patients were followed up for 8 to 25 months.The contour of reconstructed digit was as same as contralateral digit with satisfactory motion arc and sensation.There was no extensive scar in donor toe.The width of medial plantar triangle flap increased significantly.All patients could walking,running,jumping without restricted.Conclusions With reconstructed by modified great toe wrap-around flap,degloved thumb or finger can be promised with excellent contour and function outcome.In the meantime,the loss of donor foot can be expected to minimal.This procedure is one of the best ways for reconstructing degloved thumb and finger.
